[Non-functioning primary neuroendocrine tumor of the liver with extension to the celiac trunk].

چکیده

El tumor neuroendocrino primario hepático (TNEHP) es extremadamente raro, lo que supone un reto tanto diagnóstico como terapéutico. Ası́ mismo, la clasificación de su naturaleza como primario requiere un estudio diagnóstico exhaustivo que demuestre la ausencia de enfermedad a otros niveles, su confirmación histológica y seguimiento prolongado para descartar la existencia de otra tumoración primaria no diagnosticada. Se presenta un caso clı́nico tratado con éxito mediante cirugı́a radical que requirió resección de arteria hepática (técnica de Appleby). Se trata de un varón de 71 años que consulta por dolor epigástrico irradiado hacia hipocondrio derecho, acompañado de sensación de plenitud pospandrial con vómitos de pocas horas de evolución. A la exploración presenta dolor a la palpación de epigastrio e hipocondrio derecho palpando masa en epigastrio con hepatomegalia de 2 traveses. La ecografı́a abdominal mostró la existencia de una gran masa sólida que distiende y deforma el lóbulo hepático izquierdo. TC compatible con neoformación hepática, polilobulada, probablemente primitiva con dudosa extensión a cuerpo páncreas y tronco celiaco (fig. 1). Tras amplio estudio realizado durante su ingreso se descartó la existencia de enfermedad extrahepática. Se emplearon endoscopia digestiva alta, colonoscopia, broncoscopia, RMN, PET-TAC y octreoscan sin captación. El laboratorio mostró elevación del antı́geno CA 125: 62,45 U/ml, enolasa neuroespecı́fica 370 ng/ml y cromogranina A: 605 ng/ml. Gastrina, somatostatina, histamina, glucagón, somatostatina, CEA y AFP en niveles normales. Se procede a biopsia hepática percutánea con diagnóstico histológico e inmunohistoquı́mico compatible con carcinoma neuroendocrino.
